Seeing Differently After a Stroke

A stroke can have profound effects on the eyes and visual system, often leading to changes that significantly impact a person’s quality of life. Because vision depends on a complex network involving the eyes, optic nerves, and several areas of the brain, a stroke that disrupts blood flow to any of these regions can impair how visual information is processed.

One of the most common visual consequences of stroke is visual field loss, such as homonymous hemianopia, where the same half of the visual field (side vision) in both eyes is missing. For example, a stroke in the right occipital lobe of the brain may cause a loss of vision on the left side of the visual field. People with this type of impairment may bump into objects, struggle to read, or feel disoriented in crowded environments.

Another condition, known as visual neglect or spatial inattention, can occur when the stroke affects the brain’s ability to attend to one side of space—typically the left. Unlike field loss, the eyes may function normally, but the brain ignores visual information from one side. This can cause individuals to shave or eat from only one side, or to read just half of a page.

Eye movement disorders are also common after a stroke. Damage to the brainstem or areas that control eye coordination may result in double vision, difficulty focusing, or an inability to move the eyes smoothly. This can make tasks such as reading or tracking moving objects very challenging.

Some patients experience reduced visual processing speed or difficulty recognizing faces and objects. These abilities depend on higher cortical functions—the brain’s advanced processing system that combines information from what we see, remember, and understand to help us recognize, interpret, and respond to the world around us. Even small areas of damage caused by a stroke can lead to perceptual difficulties that interfere with everyday activities.

Rehabilitation plays a crucial role in recovery. Techniques include vision therapy, compensatory scanning strategies, prism lenses, and occupational therapy to help individuals adapt. While not all visual deficits can be fully restored, many patients can significantly improve with targeted therapy and consistent practice.
